[MCP] MCP server name got also cached

Steps to Reproduce:

1. In my settings.json file I updated the sse URL for my MCP server. Then I restarted it, and clicked on the tools icon. This opened up the action bar, unfortunately it still showed the old name.

New Name:

old name == mcp server: weather

It looks like you still have a weather server in your config, so that will continued to get served. Tools are cached by name, so any server named weather in your user settings will have the same cached tools, at least until it's started next time.
