there is a meeting list which has meetings mysteriously by appointment, this PR adds a test file for that site (and another one for another site)
additionally, this infers country when not present but the state in in the USA or Canada - this helps address-parsing
finally, it adds an "Appointment" option to the time dropdown when there are appointment meetings in the data - otherwise they get added to the bottom of the list and are hard to find
